The rule behind each date
The Purnima tithi is the last twelve degrees of the Moon's approach to full opposition with the Sun. A panchang names the day by the tithi at sunrise, which is the day for the Purnima snan and daan. The Purnima vrat (fast) is usually kept on the day the Purnima tithi is running at moonrise, which can be the day before. Several Purnimas carry a festival: Chaitra Purnima is Hanuman Jayanti, Vaishakha is Buddha Purnima, Ashadha is Guru Purnima, Shravana is Raksha Bandhan, Ashwin is Sharad Purnima, Kartik is Kartik Purnima and Dev Deepawali, and Phalguna is Holi. Each festival follows its own timing rule (Sharad Purnima, for example, is kept on the night the Purnima holds), so its day can differ from both columns; check the festival's page.

Why are there two dates for some Purnimas?
The sunrise day is when the Purnima tithi holds sunrise, used for snan and daan; the vrat day is when it is running at moonrise, used for the fast. When the tithi begins in the evening, the two fall on consecutive days.
Is Purnima the same date everywhere?
The tithi's start and end are the same moment everywhere. The day it holds sunrise or moonrise can differ between cities; the dates here are for New Delhi.
